How can I request a policy loan?

Click here for a Policy Loan Request form (No. 6101). Fill it out and mail it to us at the Home Office in Oak Brook, IL, or fax it to 630-684-5445.

> Policy Payments

How can I pay premiums on a policy or repay policy loans?

You can pay premiums and repay loans electronically, through our secure payment portal on the Online Policy Manager; by mail; or by having payments debited monthly from your checking account.

  • To pay through our payment portal: Login to the Online Policy Manager, using the Policyowner Login on this site. Click "Select a Policy," choose the "Financial Information" tab, click "Make a Payment," and follow the prompts. (First time visitors will need to register on the site.)

  • To pay by mail: Make all checks payable to Mutual Trust Life Insurance Company. Please include your policy number on the check and the bottom portion of your billing notice. To repay a policy loan, click here for a Loan Repayment Slip.

  • To have policy premiums and/or loan repayments debited monthly from your checking account: Click here and fill out our Pre-Authorized Payment Plan Request form (No. 2501-9).
