That's why it's important to periodically review the beneficiaries and contingent beneficiaries on all your Mutual Trust insurance and annuity policies. To make a change, simply follow these easy steps.
- Download the "Request for Change of Beneficiary" form by clicking here. Or call us at 800-323-7320, ext. 5060 and we'll send you the form.
- Fill out the form and sign it.
- Email the form to policypayments@mutualtrust.com, or fax it to 630-684-5444, or mail it to Mutual Trust Life Insurance Company, 1200 Jorie Boulevard, Oak Brook, IL 60523-2269.
